成人型アトピー性皮膚炎 (atopic dermatitis : AD) 患者8例 (男性4例, 女性4例, 平均26歳) に対して, 週に1回, 計8週間の鍼治療を行い, 神経症症状の変化について, 心身医学的検査 (Cornell Medical Index : CMI) および独自に開発した自覚的かゆみ尺度を用いて評価した。治療前に, CMIの深町の神経症判別基準において正常域にあるものは1名のみであり, 残りの7名は何らかの神経症症状を示していた。8週間の治療によって, 深町の神経症判別基準では4名が改善, 4名が不変であり, 項目別では, 身体的自覚症のうち習慣の項目が, また精神的自覚症の合計と不適応の項目が Wilcoxon 検定で有意の改善を認めた。また自覚的かゆみ尺度は, 鍼治療の前後で, 平均2.75から1.00に下がり, 全例が改善した。通常の神経症患者には, 性格的素質, 生活史上の体験, 現在の環境などの因子が関与しているが, AD患者の場合, AD患者であることによる二次性の心理社会的ストレスが複雑に絡み合って神経症症状を形成していると考えられ, この悪循環の改善の一助として鍼治療が有効であると思われた。
The authors studied the psychosomatic effects of acupuncture in patients with atopic dermatitis. The subjects were eight adult patients with atopic dermatitis, four males and four females, averaged 26 years old. They received acupuncture therapy once a week for 8 weeks. Before and after 8 week-therapy, the authors evaluated their psychosomatic symptoms by Cornell Medical Index (CMI) and the originally developed self-rating scale for itching. Before the treatment, seven patients had psychosomatic symptoms on the basis of Fukamachi's criteria of CMI and for of 8 patients showed marked improvement after acupuncture therapy. At the end of acupuncture treatment, habits-related somatic symptoms, total psychical symptoms and the self-rating scale for itching were all significantlly reduced (P<0.05) (Wilcoxon's test). The results suggest that acupuncture would effectively reduce psychosocial factors aggravating psychosomatic symptoms in patients with atopic dermatitis.
