抄録
(目的) 股関節に対する装具療法は, 観血的治療に比べて評価が低いが, 股関節疾患の重要な治療法の1つである。今回, 様々な股関節疾患に対して装具療法を施行し, 早期社会復帰・就業に成功したので, 臨床成績を報告し, 歩行分析を行った。(方法) 症例に対して, 小林らが開発した functional brace を装着し, 早期歩行・社会復帰・就業を試みた。症例は大腿骨頭壊死4例, 大腿骨頸部内側骨折2例, 股関節脱臼骨折1例である。さらに正常股関節に対して装具作製し, 歩行時荷重分析を施行した。(結果) いずれの症例も, 装具使用にて日常生活の改善・早期社会復帰が可能であった。歩行解析では, 装具の有無による, 踵部・前足部及び, 装具の坐骨支持部の荷重を測定した。装具にて, 踵部の荷重は, 1/4に軽減するが, 前足部の荷重は装具の有無に関係なかった。また坐骨支持部の荷重は, 少なく, 坐骨支持以外の支持機構が推測され, ソケットで締めつけられた大腿周囲筋の静水圧が関与していると推測した解析した臨床症例から, 免荷の度合いを推定し, 従来の装具と同等の免荷がなされていると推定した。(結論) functional brace は, 股関節疾患患者の早期社会復帰を容易にする。また, functional brace は, 活動性が高く, 装着の苦痛も少ないことから, もっと多用してよいと考える。
The surgical treatment of hip disorders has progressed significantly over the past two decades, but the treament with orthoses has not. We indicate that patients with hip disorder can return to activities of daily life with wear of a functional brace and measure static forces in the brace in gait. Seven patients with hip disorders were managed with functinal braces. Of these patients, the etiology of the hip disorders was osteonecrosis in four, femoral neck fracture in two, hip dislocation in one. One case of traumatic osteonecrosis wore the functinal brace and returned to work without a cane and pain medication in three years. The other osteonecrosis of traumatic etiology sustained a hip dislocation with femoral head and acetabular fracture. After operation he began walking with the functional brace. A minimal osteonecrsis appeared but did not get worse. The patient with idipathic osteonecrosis was traeted with the functional brace. Within a yaer the amount of pain medication was decreased and joint space was slightly wider. The patient with steroid induced osteonecrosis can return to his sailor occupation with the funectional braces in five month. The patient with a fracture-dislocation of hip can return to his job within four month with the functional brace. Another two patients with femoral neck fracture can return to home with the functional brace earlier. Static forces were measured in gait with or without a functional brace. In heel strike, the force at the calcaneal area with the functional brace is 25% less than normal. The force at the forefoot with the functional brace is equal to normal. The force at the sciatic area with the functional brace is little. So we think that the functional brace suspends the hip by hydraulic pressure support.
