精神科デイケアの長期利用者の実際の参加状況と意識から、長期利用に対する背景を明らかにし、今後への対応を検討した。対象は、単科精神病院付設デイケア通所中の長期利用者6人(男4人、女2人)である。方法は、個別面接を行いその内容を逐語録にし、関係するコメントを抜き出し、カルテなどから得た基本情報やエピソードとあわせて検討した。その結果、長期利用者は、デイケアを「仕事」「健康管理」「余暇活動」「生活維持のための援助」のために利用していることが明らかとなった。今後、医療専門職がもっているリハビリテーションのノウハウを、地域の社会資源のスタッフに伝えること、ネットワークを築<ことによって情報・人的・プログラム基盤を整え、社会資源の能力を高めることが必要である。また社会資源の充実には経済的基盤も不可欠である。このような結果として、地域の社会資源のイメージが改善すれば、デイケアの長期利用者の一部が地域社会資源を利用できる可能性があることが示唆された。
What causes those who stay in a psychiatric day service program for a considerable time unable to move on to a next step to be back into their community? I surveyed the perception of the clients whose inclusion to our day program more than 5 years. The individual interviews of the 6 long-term clients (4 males, 2 females) were transcribed verbatim and the comments related to the day service were extracted. These data were studied carefully along with the information within the patient file. The long-term clients have attended the day service as their 'work', 'health control', 'diversion' and 'to receive a support for disability in life skills.' Medical professionals could introduce their knowledge on psychosocial rehabilitation and establish a human supportive network for each client. A capability of a social support system would be improved by organizing information, human resources and a disability support program in a community. One cannot deny the importance of a stable economical resource to enhance a community program. Our survey showed that improvement of the image of community resources for disability support will promote some ofthe long-term clients of our day service move onto their community programs.
