癌細胞増殖能の評価はすでに癌の予後規定因子のーっとして確立され,さらに血行性転移との相関も示唆されている。しかしながら,血行性転移の前段階としての局所浸潤能と癌細胞増殖能との関連は明らかでない。胃癌53例を対象としてurokinase-typeplasminogen activator (u-PA)およびtissue-typeplasminogen activator (t-PA)とそのinhibitorであるplasminogen activator inhibitor-1 (PAI-1)を免疫組織学的に検討し,同時に細胞増殖能をAgNOR scoreで評価することで,相互の関連について考察を加えた。u-PA・t-PA・PAI-1はそれぞれ45.3・52.8・69.8%に陽性であり,臨床病理学的所見との関連ではu-PA陽性例では陰性例よりも静脈浸襲が高い傾向を認め,t-PA ・PAI-1は未分化型に比較して分化型で有意にl高い陽性率を示した。癌細胞増殖能との関連ではu-PA・PAI-1ともに陽性例は陰性例に比較して有意に高いAg NORscoreを示し,PAで代表される局所浸潤能とAgNORで代表される細胞増殖能とは関連を有しており,相互に影響しあって癌腫の発育進展を助長しているものと考えられた。
Proliferative activity of cancer lesion has been already confirmed as one of the prognostic indicators in cancer patients. Furthermore, high grade of proliferative activity has been reported to be related to hematogenous metastasis. Although, the relationship between Proliferative activity and proteolysis around cancer lesion which was considered to be the first step of hematogenous metastasis has not yet been clarified. In the current study, 53 cases of gastric carcinoma were examined the expression of urokinase- and tissue-type plasminogen activator (u- and t-PA) and plasminogen activator inhibitor-1 (PAI-l) immunohistochemically. Relationships between their expression and clinicopathological findings were studied, and their relations to the proliferative activety were evaluated using the score of stained argyrophilic nucleolar organizer regions. Expression rates of u-PA, t-PA and PAI-1 were 45.3, 52.8 and 69.8% respectively. As to the clinicopathological findings, rate of venous invasion was tended to be higher in u-PA positive cases and the expression rate of t-PA and PAI-1 were significantly higher in differentiated carcinoma compared with the undifferentiated carcinoma. Concerning the proliferative activity, u-PA and/or PAI-1 positive groups showed significantly higher AgNOR scores compared with negative groups. It was suggested that the proteolysis induced by PA was closely related to the proliferative activity of cancer lesions and that they might accelerate the turn or extension together with each other.
