虚血性心疾患の病因,病態におけるproteinC (PC)抗凝固系の関与を検討する目的で,血漿PC抗原量,PC活性およびPC inhibitor (PCI)活性を22名の安定狭心症患者(労作狭心症10名,安静狭心症12名),9名の不安定狭心症患者および18名の相応する年齢の対照者(健常者10名,胸痛症候群患者8名)で測定した。安定狭心症患者では,PC抗原量とPCI活性は対照群より有意な高値を示した。一方,PC活性は対照群と有意差を認めなかった。不安定狭心症患者ではPC活性とPCI活性は,不安定期には安定期より低値を示した。PC抗原量は不安定期の方が低い傾向を示したが有意差を認めなかった。以上の結果より,安定狭心症患者では血液凝固機能の亢進によって血中PC-PCI結合体の増加がみられ,不安定期にはPCが急激に消費されていることが示唆された。
To examine the protein C (PC) anticoagulation system in coronary artery disease, PC antigen values, PC activities and heparin-dependent PC inhibitor activities in the blood were determined in 22 patients with stable angina pectoris (AP) including effort AP (n=10) and rest AP (n=12), 9 patients with unstable AP and 18 age-matched control subjects. PC antigen values and PC inhibitor activities in patients with stable AP were significantly higher than those in control subjects (p<0.01). In contrast, there was no significant difference in PC activities between these patients and control subjects. In patients with unstable AP, PC activity and PC inhibitor activity were markedly lower during active period than in their stabilized period. These results may be at least partly due to increased circulating PC-PC inhibitor complex, which has no acfivity but has antigenicity, refleting exaggerated thrombin activity and hypercoagulability in stable AP and rapid consumption of PC during active unstable period in unstable AP.
