精神分裂病発病後に強迫症状が出現した5例について縦断的な観察を行ない,精神病理学的検討を加えた。強迫症状の出現は急性期が終わった後の不安定な時期が多かった。多くの症例で,先行する幻覚や妄想の残滓が強迫観念の機能的部分に入り込み,強迫神経症とは異なる独特な病像を示した。著者の症例において強迫行為は,確認強迫あるいはその変形である質問強迫(保証強迫)であり,周囲を巻き込むことにより,内的葛藤を患者と周囲の人物との間の葛藤に転化する傾向が見られた。このようにして獲得した対人関係は,反復,紋切型化,儀式化を免れず,回復過程の停滞の原因ともなっていた。すべての症例において一過性に身体表象の異常が存在することは,強迫症状が身体の崩壊感覚に対する防衛として出現した可能性を示唆していると考えた。強迫症状の出現後,一般に病像は慢性化・固定化する傾向にあったが,精神分裂病のはなぱなしい再燃はなく,著しい人格の解体は見られなかった。
Five patients who developed obsessive-compulsive symptoms after the onset of schizophrenia were studied with psychopathological approach. In most cases, the obsessive-compulsive symptoms occurred at an unstable post-psychotic phase. The patients repeated compulsive doubt that demanded assurance. They seemed to try to control their environment in order to reduce their internal conflicts. Unlike obsessive features of neurotics, those of the schizophrenic patients were often mingled with fragmented hallucinations and delusions. All of them felt abnormal sensations about their bodies at least transiently. It seems that their obsessive-compulsive interpersonal behaviors emerged as a defense mechanism against their collapsed body image. Obsessive-compulsive symptoms tended to consolidate schizophrenic features into chronic phase and to delay recovery from the illness. However, the patients have never relapsed and been relatively free from degradation of their personality.
