関心術症例65例を対象とし,Cell Saverを用いた回収式自己血輸血法の有用性を臨床的に検討した。術野出血および体外循環回路残留血をCellSaverを用いて遠心分離,洗浄処理し,体外循環終了後に洗浄濃厚赤血球液として返血した。Cell Saver使用症例35例を2群に分けI群は完全無輸血手術を行い得た8例,II群は同種血輸血を必要とした27例とした。III群はCell Saver非使用症例30例で,これを対照として比較検討した。返血された洗浄濃厚赤血球液は平均897mlで,平均Htは51.6%であった。CellSaver使用症例35例中8例(23%)において完全無輸血関心術が達成され,輸血例においても平均34%の同種血が節減された。これによって輸血後非A非B型肝炎の発生率は,Cell Saverを用いなかった場合の20.2%から7.4%に低下した。CellSaver使用例における術後出血量の増大は認められず,凝固線溶系の変動も非使用例と差がなかった。さらに,Cell Saverによる同種血輸血節減は,腎機能庇護の面からも有用であり,かつ術後早期の免疫能の低下も認められなかった。
Sixty-five patients were studied to evaluate the role of intraoperative autotransfusion in cardiac surgery. The blood recovered from the surgical field and the residual blood in the cardiopulmonary bypass circuit were centrifuged by using Cell Saver and retransfused as packed red cells after completion of cardiopulmonary bypass. Cell Saver was used in 35 patients, of which 8cases required no homologous blood transfusion (Group I). In the remaining 27 patients, some units of banked blood were transfused intra- or postoperatively (Group II). Thirty patients in which Cell Saver was not used were compared with Group I and IT as control (Group III). The amount of washed red blood cells obtained by Cell Saver and retransfused was 897 ml in average per case with an average hematocrit of 51.6%. The data indicate that (1) banked blood transfusion was diminished by 34 percent by the use of Cell Saver (1448±652ml in Group II vs 2184±1223 ml in Group III); (2) the incidence of posttransfusion non-A, non-B hepatitis was 0% in Group I, 7.4% in Group II and 20.0% in Group III; (3) postoperative bleeding was not increased with the use of Cell Saver; (4) blood curtailment with Cell Saver was beneficial to keep the patient in good renal function postoperatively; (5) Cell Saver did not alter the immune response in the early phase after the operation.
