偏光顕微鏡を用いて左室心筋の複屈折性を調べた。対象は術中心筋生検が施行された関心術症例23例(弁膜疾患21例,ASD2例)である。心筋保護法として全例St.Thomasa液によるcoldcardioplegiaとiceslushによる局所冷却法の併用を行なった。先ず,大動脈遮断前に採取した心筋標本について,ATPとCaを含んだ試薬に対する複屈折の増加量をB-test値と定め,術前心機能の評価を行なった。次いで大動脈遮断解除前に採取した心筋標本のB-test値を計測し大動脈遮断前のB-test値に対する%比を%B-test値と定め,術中心筋保護効果を検討した。B-test値はMSで4.5nm,ASDで4.1nm,MRで3.3nm,ARで3.7nmで,左室に負荷のかかるAR,MRは負荷のかからないMS,ASDに比し低値をとる傾向を示した。%B-test値では23例中,85%を越える例は19例で,85%以下は4例であった。%B-test値は大動脈遮断時間の延長に伴い低下する傾向にあり,%B-test値が85%以下の4例中3例は術後高度の不整脈あるいはLOSを生じたのに対し,%B-test値が85%を越える症例では全例術後経過は順調であった。以上の結果よりB-test値が4nm程度では左室心筋障害が少ないと考えられ,また術中心筋障害に関しては%B-test値が85%以上であれば軽度であるが,それ以下であれば高度である可能性があり術後LOS,不整脈などの発生に注意が必要である。
Birefringence test was introduced to cardiac surgery for evaluation of the preoperative condition of the myocardium and the efficiency of intraoperative myocardial protection during openheart surgery. Left ventriculara biopsies were performed in 23 patients, under extracorporeal circulation, before and after aortic cross-clamping. Of these 23 patients, 21 patients underwent valve replacement and 2 had ASD closure. Changes of birefringence of biopsied specimens by adding ATP-calcium buffer were measured by quantative polarization microscopy, and these changes were defined as B-test. B-test values were 4.5nm in MS, 4.1nm in ASD, 3.3nm in MR, 3.7nm in AR and 3.6nm in CVD. The B-test values of the patients with MR or AR showed lower values than those of the patients with ASD or MS, possibly because of volume lord on the left ventricle in the former groups. We calculated the B-test value after aortic crossclamping and defined its percent of the preaortic cross clamp value as %B-test. All patients with more than 85% of %B-test showed uneventful postoperative course, but 3 out of 4 patients with lower value than 85% showed postperative complications, such as LOS or frequent PVCs. It is concluded that birefringence test is a useful method to evaluate the myocardial function and the efficiency of intraoperative myocardial protection.
