難治性網膜剥離であるProliferative vitreoretino-pathy(以下PVRと略)は,剥離網膜前後面にそれぞれPreretinal membrane(以下PRMと略)とRetroretinal strand (以下RRSと略)ができ,それらが収縮するために生じる。今回筆者は26例のPRMと6例のRRSを電子顕微鏡で観察した。またシリコンオイルは重症のPVRに眼内充填剤として用いるのに有効であるが,シリコンオイル充填眼より得られた12例のPRMと1例のRRSが32例の中に含まれる。PRMの構成細胞は網膜色素上皮細胞,グリア細胞,大食細胞,線維芽細胞,筋線維芽様細胞が報告されているが,今回の検討では筋線維芽様細胞を除くすべての細胞が観察された。大食細胞は網膜色素上皮細胞より分離される所見が観察された。線維芽細胞は間葉系細胞と推察されている報告が多いが,網膜色素上皮が線維芽細胞様に異化する過程が観察された。RRSの構成細胞としてグリア細胞の明確な病理組織像を呈示した報告は筆者の知るかぎりないが,グリア細胞からなるRRSが存在した。シリコンオイル充填眼のPRMとRRSでは,網膜色素上皮細胞と大食細胞にシリコンオイルの取り込みがみられ,グリア細胞にはほとんど認められなかった。また血管の増生が顕著で,シリコンオイルの機械的刺激が考えられた。
This report dealed with histopathologic and ultrastructual features on 26 preretinal membranes and 6 retroretinal strands in proliferative vitreoretinopathy. They included 12 preretinal membranes and 1 retroretinal strand removed from the eyes which were tamponaded by silicone oil. It was already reported that preretinal membrane formed from pigment epithelial cells, glial cells, macrophages, fibroblasts and myofibroblast like cells. All these cells except for myofibroblast like cell were included and discussed in this study. Macrophages were found to be derived from pigment epithelial cells. It was usually said that fibroblast was mesenchymal origin in preretinal membrane and retroretinal strand. But fibroblasts were observed as the transformation of pigment epithelial cells, so fibroblast was suspected to be derived from pigment epithelial cell. In litereture to date there were no histologic features of glial cells in retroretinal strand, but in this series one retroretinal strand which was consisted of only glial cells was observed. Macrophages and pigment epithelial cells engulfed silicone oil in preretinal membranes and retroretinal strand removed from the eyes which were tamponaded by silicone oil, but glial cells did few. Futhermore, many vesseles were observed in these membranes and strands in comparison to those from the eyes which were not tamponaded by silicone oil, because silicone oil seemed to stimulate them.
