抄録
蛍光抗体法によりメサジジウムにびまん性にIgAの沈着がみられ,全身性疾患の合併のない事が確認できた154例の小児IgA腎症を対象に糸球体の電顕的検討を行なった。メサンジウムのelectron-dense deposits (EDD)は8例を除く全例に観察された。糸球体基底膜(GBM)の上皮下のEDDは40例にみられ,大部分にメサンジウム及び内皮下のEDDを伴なっていた。上皮下のEDDは臨床的に重症な症例,進行性の経過をとる症例,光顕で糸球体変化の強い症例に多くみられた。また61例に再生検を行ない,再生検時尿所見か正常化していた症例では,メサンジウム,内皮下,上皮下のEDDの減少がみられたが,尿所見の持続していた症例ではEDDは持続していた。これらの所見より小児期IgA腎症ではその進行にEDDの存在が密接に関与していると考えられた。
An electron-microscopic study of the glomeruli was made on 154 children with IgA nephropathy. IgA nephropathy was diagnosed when immunofluorescence microscopy showed diffuse mesangial deposition of IgA and there was no evidence of systemic disease. Mesangial electron-dense deposits (EDD) were observed in all but 8 children. Subepithelial deposits were observed in 40 children and were almost always accompanied by both mesangial and subendothelial deposits. Sudepithelial EDD were significantly associated with more severe clinical presentations. a worse outcome and more severe light microscopic glomerular changes. In 61 patients sequential pathologic observations were made. The patients whose urinary findings were normal at the time of renal biopsies showed decrease of the EDD in mesangial. subendothelial and subepithelial regions between the two biopsies. However those who had persistent proteinuria and/or hematuria at the time of repeat renal biopsies showed no difference about the EDD behTeen the two biopsies. These findings suggest that the EDD are significantly associated with the progression of IgA nephropathy in children.
